65 years old, male.
Diagnosis: 1.9cm perforated gastric ulcer. He is post operation for Laparotomy
exploratory Slip, which was performed on him in this present hospital.
Health history: GERD, hypertension and HLD presents by EMS transfer form
from previous hospital for perforated gastric ulcer with free air fluid. Prior
records showed patient was seen at prior facility yesterday endorsing abdominal
pain after eating Mexican food. patient had BP of 70/50 and was given
versed, fluids, diilaudid and Zosyn prior to transfer. Patient reports
abdominal pain currently but denies NVD, fever, chills, hemoptysis, backpain,
CP, SOB.
Prior labs and Imaging: WBC 12.7
CT ABD/PELV/CHEST IMPRESSION: 1.9cm perforated gastric ulcer at the gastric
antrum with inflammatory changes throughout the stomach and distal esophagus.
small volume intra-abdominal free and free fluid.
